Client Communication 6-17-2020

Dear Clients:

I know that many of you are as tired of social distancing and face masks as I am, but the coronavirus is still spreading in our community, so our doctors and staff are doing our best to balance the needs of our clients and patients with our responsibility to keep each other safe. We are providing a full range of services, but with the following limitations:

General Procedures

1. If you or anyone in your household answers yes to any of the following questions, we ask that you do not come here. If your pet has a medical emergency, send a family member or friend who can answer no to these questions:

· In the last 14 days, have you traveled outside the country?

· In the last 14 days have you experienced any of the following symptoms:

o Fever over 100 degrees

o Persistent cough

o Difficulty breathing

· In the last 14 days have you tested positive to the COVID-19 virus?

2. Call before coming so we expect you and are ready for you.

3. Consider our Drop off service- Drop your pet off for services and pick them up later

4. Consider our Telemedicine service- schedule a phone consultation with one of our doctors for minor problems. There is a fee for this, but it may save you from having to bring your pet in. 5. Before coming for appointments, we recommend that you go to our web site (www.animal-medical-clinic.com) to print and complete the following forms, if applicable:

· New Patient Form

· Risk Factor Questionnaires- for all preventive care exams

· Medical drop off- all sick patients

· Standard consent- all procedures except dentistry

· Dental consent

· Boarding Registration

Outpatient appointments

1. Some doctors are allowing one client, with face mask, into the exam room with the pet. Some are not allowing clients into the exam room, either due to their physician’s orders or their personal comfort level. If you have a strong preference, let us know when scheduling the appointment.

2. When you arrive, park in one of our numbered parking spots and wait in your car. If you are not greeted by one of us within a few minutes, call us at 321-724-2277

3. If you are coming in with your pet, we will let you know when we are ready for you. Only one person may accompany the pet. You must wear a face mask, and we will scan your temperature with a touchless infrared thermometer. You may be asked to wait temporarily in our waiting room which is set up for social distancing while the exam room is being cleaned. When the appointment is finished, we will check you out in the exam room and you can go straight back to your car.

4. If you are not coming inside with your pet, histories will be taken in writing, on the phone, or outside (at a distance). Your pet will be brought inside for exams, tests and treatments. The doctor will call you to discuss findings and recommendations
